I'm just curious. What is the native Vlan on the switch for your 802.1q trunking (the one that is untagged)? As I am assuming Vlan3 your voice one is not going to be the native VLAN.

No, the voice VLAN is not the native VLAN, it happens to be the access VLAN for right now until we setup a separate VLAN just for trunk links.

Try setting up a new DHCP server in a Cisco switch or Router. See if the phone acquires an IP address. Use the debug command to sniff DHCP broadcasts. You must check if the broadcast are arriving to the server.

Hey something just came to mind when I was working with one of my clients on their Voice Vlan. Do you have CDP enabled on the phone and on the Switch? That is how the switch discovers the phone.

Check the port configuration of the the switchport connected to the DHCP server.

I've found that if they have voice vlans or trunk configurations applied, the Windows DHCP server will respond to traffic that wasn't meant for it, or respond on the wrong vlan. Set it up as a basic access port, and see if your vlan problems don't go away.

The switch is not directly connected to the DHCP server. It uses a virtual interface to contact it and with a helper-address.

sniffing traffic of phone / pc by a span port on the switch isn't a possibility?
